Oregon delivers Medicaid through Coordinated Care Organizations (CCOs), a state-invented model created by 2012 legislation that's genuinely unique nationally. Each CCO is locally governed, with a Community Advisory Council made up of at least 51 percent Medicaid members, and operates on a single integrated global budget covering physical, behavioral, and oral health together, rather than carving any of them out separately. CCOs can also spend on Health-Related Services, non-traditional supports like housing and food assistance, when it helps a member's health. Fifteen CCOs currently serve nearly a million Oregon Health Plan members, and most are geographically defined, so which CCO a patient has often depends on which specific counties a practice serves.
On the commercial side, Oregon's market is in real flux. Providence Health Plan, the state's third-largest insurer with more than 421,000 members, announced in May 2026 that it's exiting most of its Oregon insurance business for 2027, citing rising costs and regulatory pressure. PacificSource has also announced an individual-market exit for 2027. That leaves Regence BlueCross BlueShield and Kaiser Permanente, historically near-tied for the top spot, as the state's most stable options, with Kaiser's integrated HMO model concentrated in the Portland metro area.
