Regional Accountable Entities (RAEs) coordinate care and administer the capitated behavioral health benefit for members in their region, but most physical health fee-for-service claims are still paid directly by the Department of Health Care Policy and Financing (HCPF), not by the RAE. Two full-risk managed care organizations operate as exceptions to that fee-for-service default: Denver Health Medicaid Choice and Rocky Mountain Health Plans PRIME, which require providers to bill the MCO directly rather than HCPF.
On the commercial side, Kaiser Permanente is Colorado's dominant carrier by enrollment, offering HMO-only coverage with no PPO option. For patients who want out-of-network flexibility, only Anthem Blue Cross Blue Shield and Cigna offer PPO plans in the individual market.
