MaineCare is one of only four state Medicaid programs with zero percent managed care, using a direct fee-for-service model instead. Providers enroll once through DHHS and the MIHMS system, and there's no separate MCO credentialing to manage the way there is in most states. MaineCare reimburses most Medicare-covered services at approximately 72.4 percent of the Medicare rate, and about half of beneficiaries participate in a Primary Care Case Management program that adds a monthly case management fee on top of standard fee-for-service payments.

On the commercial side, Anthem is Maine's dominant insurer, covering more than half of CoverME.gov marketplace enrollees and holding the broadest provider network in the state, including partnerships with both MaineHealth and Northern Light Health, the state's two largest hospital systems. That relationship isn't always smooth: in 2022, a payment dispute nearly caused MaineHealth to drop Anthem from in-network status at Maine Medical Center before the two settled on a new contract. Maine's Bureau of Insurance also requires standardized "Clear Choice" plan designs, so cost-sharing is identical across carriers within the same metal tier.
