Kansas delivers nearly all Medicaid services through mandatory managed care under KanCare, administered jointly by the Kansas Department of Health and Environment for financial oversight and the Kansas Department for Aging and Disability Services for disability waivers, mental health programs, and long-term care. On January 1, 2025, Kansas launched KanCare 3.0, replacing its prior MCO lineup with new contracts running through December 31, 2027, for three plans: Healthy Blue (a new entrant), Sunflower Health Plan, and UnitedHealthcare Community Plan of Kansas. Every KanCare enrollee is assigned to one of these three MCOs, and the program currently covers roughly 430,000 to 458,000 Kansans depending on the reporting period.

A small share of services still run through fee-for-service, billed directly to the state's KMAP portal, but the large majority of members receive care through their assigned MCO, which must meet the state's minimum floor rate even though each plan negotiates its own contracted rates above that floor. Kansas has not expanded Medicaid under the ACA, which leaves an estimated 28,000 childless adults earning below the federal poverty level with no path to KanCare and no marketplace subsidy either, since ACA subsidies only begin at 100 percent FPL. Throughout 2026, KanCare is rolling out a Community Care Coordination requirement, under which MCOs must conduct social determinants of health screenings and connect members to food, housing, and transportation resources as part of standard care coordination.
