Nevada's Statewide Managed Care Expansion and Its Regional Payer Split
Nevada expanded Medicaid in 2014 under then-Governor Brian Sandoval, the first Republican governor to commit to ACA expansion, and now covers roughly 738,000 residents. Until this year, managed care was mandatory only in urban Washoe and Clark counties, with rural residents remaining on fee-for-service. That changed in January 2026: managed care expanded statewide for the first time, moving approximately 75,000 rural residents into MCOs and marking Nevada's biggest Medicaid change since the original expansion. SilverSummit Healthplan now covers all 17 counties, and CareSource joined as a new statewide plan option.
On the commercial side, Health Plan of Nevada (owned by UnitedHealthcare) leads the individual market with about 61 percent share, making Nevada the only state in the country where UnitedHealthcare holds the top individual-market position nationally. Clark County, home to Las Vegas and roughly 73 percent of Nevada's population, is dominated by Health Plan of Nevada and Anthem affiliates, while Hometown Health, owned by Renown Health, the region's dominant hospital system, leads in Washoe County around Reno. Nevada's marketplace is otherwise genuinely competitive, with eight to ten carriers for 2026 and a new state-run "Battle Born State Plan" option that launched in late 2025.
