Same Insurer, Different Rules: Anthem's Dual Role in Indiana
Indiana delivers Medicaid through four managed care programs: Hoosier Healthwise for families and children, the Healthy Indiana Plan (HIP) for low-income adults, Hoosier Care Connect for aged, blind, and disabled members, and Indiana PathWays for Aging for seniors. MDwise exited the program on January 1, 2026, and its members were redistributed among the remaining managed care entities: Anthem, CareSource, Managed Health Services (MHS), UnitedHealthcare, and Humana. All Indiana Medicaid managed care claims carry a 180-day timely filing limit.
On the commercial side, Elevance Health (Anthem) controls roughly 68 percent of Indiana's commercial insurance market and is also the largest Medicaid managed care entity in the state, which creates a genuinely unusual billing situation: the same carrier, but with strictly separate commercial and Medicaid workflows, rules, and claim formats. Indiana Senate Bill 480, effective July 1, 2025, also reduced prior authorization requirements for fully-insured commercial plans.
