Idaho's Shift From Healthy Connections to Managed Care
Idaho is in the middle of the biggest change to its Medicaid program in decades. For years, Idaho ran one of the least managed-care-reliant Medicaid programs in the country through Healthy Connections, a primary care case management model that kept most services under direct state control. Healthy Connections ended on December 31, 2025, and Idaho Medicaid Plus (IMPlus) took over as the primary delivery structure starting January 1, 2026, mandatory in 34 of Idaho's 44 counties, with a full statewide transition to managed care planned by 2029. Idaho also expanded Medicaid through a 2018 voter ballot initiative rather than legislative action, one of only a handful of states where voters made that call directly.
On the commercial side, Blue Cross of Idaho, the state's only Idaho-based insurer, is the dominant carrier with roughly half the market and a network covering 96 percent of Idaho physicians across all 44 counties. Regence Blue Shield, which administers the state employee health plan, is the next largest. Idaho's individual market is unusually competitive for its size, with eight carriers, more than many states its population would suggest.
